Disability claims, it depends on whether we’re talking about an employer-sponsored plan, so my answer is going to be limited to plans that your employer offers to you as an employee that you pay for or your employer pays for during your employment. If you go out on disability leave and that disability is denied and you’re not paid, you have a right to an internal review, and then you have another right to appeal, and one more: ultimately you can even go as far as filing a lawsuit. The answer to the question how many times can you appeal it: it depends on the type of denial, but in general at least three, but there could even be more.
